IMMUNIZATION CERTIFICATION STATEMENT
In accordance with Idaho law and district policy, at the time of first admission to any school within this district, and before attendance, all students must present an immunization record regarding the child’s immunity to certain childhood diseases. The record, signed by a physician, a physician’s representative, or another licensed health care professional, shall verify that the child has received or is in the process of receiving the immunizations as specified in Idaho Code §39-4801(2) and this policy, or can demonstrate, through verification in a form approved by the Idaho Department of Health and Welfare, immunity gained through prior contraction of the disease. Alternatively, at the request of the parent or guardian, the district will access the voluntary statewide registry of immunization status (IRIS) maintained by the Idaho Department of Health and Welfare to verify whether the student has received the required immunizations.
IMMUNIZATION CRITERIA
The manner and frequency of administration of age appropriate immunizations shall conform to recognized standard medical practices in Idaho. Students are required to be immunized as follows:
1. All children require the following:
a. Diptheria, Tetanus, and A-Cellular Pertussis (DTaP) vaccine, five (5) doses;
b. Hepatitis A vaccine, two (2) doses;
c. Hepatitis B vaccine, three (3) doses;
d. Measles, Mumps, and Rubella (MMR) vaccine, two (2) doses;
e. Polio vaccine, four (4) doses; and
f. Varicella vaccine, two (2) doses.
2. In addition to the vaccines identified in subsection 1 above, the following must be met for admission into the seventh grade:
a. Diptheria, Tetanus, and A-Cellular Pertussis (DTaP) Booster, one (1) dose; and
b. Meningococcal vaccine, one (1) dose.
3. In addition to the vaccines identified in subsections 1 and 2 above, prior to admission into the twelfth grade, students who received their first dose of meningococcal vaccine before sixteen (16) years of age must have two (2) doses of meningococcal vaccine, and students who received their first dose of meningococcal vaccine at sixteen (16) years of age or older, or those who have never received a dose, must have one (1) dose of meningococcal vaccine.
EXEMPTION FROM IMMUNIZATION
Any minor child whose parent or guardian supplies documentation to the district at the time of admission and before attendance of one (1) or both of the following conditions is not required to receive the required immunizations:
- Life or Health Endangering Circumstances. A signed statement of a licensed physician that the physical condition of the child is such that administration of all or any of the immunizations would endanger the life or health of the child.
- Religious or Other Objections. A signed statement of the parent, custodian, or legal guardian stating their objections on religious or other grounds. The signed statement should include: (a) the name and date of birth of the student; and (b) their student is exempt from immunization requirements under Idaho Code §39-4801(6) and this policy.
Any student of majority age who submits a signed statement to school officials stating the student’s objections on religious or other grounds shall be exempt from any or all immunization criteria at district schools.
EXCLUSION FROM SCHOOL
Students not in compliance with this policy upon first admission in preschool or in kindergarten through grade twelve (K-12) will be denied attendance, unless the student is excepted or exempted as provided herein. Any student denied attendance will not be allowed to attend any schools within this district until he or she is in compliance with this policy. Nothing in this policy limits the authority of the Idaho Department of Health and Welfare from excluding any exempted student in the event of a disease outbreak.
REPORTING REQUIREMENTS
A copy of a report of each school’s immunization status, by grade, will be submitted to the Idaho Department of Health and Welfare on or before November 1 of each school year. Such information will be reported on an Idaho Department of Health and Welfare form or electronically and will include:
a. Inclusive dates of reporting period;
b. Name and address of school, district and county;
c. Grade being reported and total number of students enrolled in the grade;
d. The name and title of the person completing the report form.
e. Number of students who meet all of the required immunizations criteria;
f. Number of students who do not meet the specified number of immunizations listed by specific immunization type;
g. Number of students who do not meet the immunization criteria but are in the process of receiving the immunizations; and
h. Number of students who claimed exemption to the immunizations.
NOTICE TO PARENTS
School officials will describe the exemptions set forth in Idaho law, with a specific citation to Idaho Code §39-4801, in any communication to parents and guardians regarding immunization.
